    • If you registered your DD 214/military discharge document with the County Clerk, you can obtain a CERTIFIED copy from their office free of charge.

Contact their office at (254) 933-5165 or request a certified copy by obtaining the DD 214 Application Form online at
      http://www.bellcountytx.com/county_government/county_clerk/military_discharge.php

You may also fax your DD 214 Application Form with a COPY of your photo ID to:  (254) 933-5176 or you may send your request by mail to:

If you did not register your DD 214 with the County Clerk's office, you may request your DD 214 from National Personnel Records Center at
http://www.archives.gov/veterans/evetrecs to download the SF Form 180

You can send your completed request (SF Form 180) by fax to:  (314) 801-9195
