The Request for Ballot by Mail Form can be downloaded from the link below.  Make sure to print the ballot form with the printer setting of Landscape due to the width of the document.  Complete all information on the form, read instructions very carefully, print or type.  Mail the completed form to:

NOTE:  March 27, 2012 is the last day the Bell County Clerk's Office can accept a request for a ballot by mail.
